If you are ever in need, we are more than happy to schedule emergency services outside of our usual hours. We believe communication is important and that we are here to take care of you. Therefore, we will notify you of treatment needs and expenses in advance. We will also assist in filing insurance and ensuring that you are given the best price possible. Additionally, we offer flexible payment plans. We are here to serve you, so please do not hesitate to contact us over any issue.
Please take a moment to print out our new patient forms on our website, www.drfayhu.com, under patient resources, so that you may fill them out at your leisure prior to your appointment. We will also have them available at the office. Please arrive 15 minutes early so that we may review your completed forms with you.
If you have dental insurance, please bring a completed insurance form or an insurance card to your first visit so that we may have a copy to keep in your chart. Once we have the correct information on our computer, it is not necessary to bring insurance forms to subsequent visits, unless you have a change in your employer or insurance coverage.
Also, for your benefit, we request your previous dentists’ information, so we can obtain your latest x-rays. If you should have any questions, please do not hesitate to call.
